Directions for Assessment:
Record your response to each question as honestly and realistically as possible using the following scale: 3—Consistently; 2—Usually; 1—Sometimes; 0—Never
Using the Answer sheet at the end of the questionnaire, record your response in each block whose number corresponds to that number statement in the questionnaire.
It is important that your answers be an honest assessment of who you are, not who others think you are, not who think you should be or who you would like to be someday.
